Upholstery foam. What do I use to cut it? Plain scissors, utility knife, something else? Also, what kinds of glue can be used on it? Would fabric glue or hot glue work?

I would suggest a sharp knife, like some sort of X-Acto. I would be worried about scissors doing something weird. Not sure about glues.

I think hot glue would melt it. But maybe not. I really have no suggestions considering the mess I made of son's Keyblade for his Sora costume. Fabric glue maybe? Or would that soak into the foam? Glue fabric to fabric rather than right to the foam because it seams to me that the foam is covered when used in pillows and furniture rather than glued to anything. Not helping, I know.

If you've got scrap bits or can afford to waste some, you could always try experimenting a bit.

What Luna said about cutting. As for gluing, there should be at least one brand of glue at a local fabric and/or craft store that is viable with foam. Just remember to always test any glue on scraps first (foam and fabrics), just in case it doesn't work out or does something really funky or disastrous.

EDIT: Ha ha. Looks like I was beat to the scrap advice! :)

Heh, yeah, like the time I spraypainted styrofoam balls and they shriveled up and basically disappeared. :P

Eek, yeah. Spray paint eating/dissolving foam is one thing that is no fun to discover by accident.

I was thinking a box cutter for the big stuff, then something more precise to whittle it into the final shape. I would think scissors would work for thinner ones, though...

I was planning on sewing wherever I could, then gluing down on parts (like on the head) where it's small pieces that aren't easy to sew.

Haha. Yeah. I think I learned my lesson from making my Disney pin case. I was hot-gluing felt sheets into sheet protectors that had their centers cut out. The glue held them together, but it was melting the plastic too.

If those kinds of glue doesn't work, use epoxy. :D That shit is hella strong.
